CC2H program keeps patients at home connected to care-givers

Leveraging the eShift® technology platform developed by Sensory Technologies, eRehab is built around an interprofessional team that places specialized therapy technicians in the home under the guidance of a directing therapist from a remote location. The therapist is able to provide real-time direction to several technicians at once with the use of Sensory Tech’s electronic health tools.

eShift® – Creating a Virtual in Home End of Life Care Team

The Palliative Care eShift® (eShift®) program is a model of care that allows for personal support workers (PSWs) connected virtually to nurses through Smartphone technology, to provide palliative clients the end of life care they need. The South West Community Care Access Centre (CCAC), with support from the South West Local Health Integration Network (SW LHIN) is  the co-ordinators of care for the eShift® model.
